Pursuant to the terms of the Click Me to Edit Donor_Advised Donor Advised Fund established at the Jewish Community Foundation, Inc., I hereby recommend that grants be made to the following 501(c)(3) organizations and/or agencies in the amounts indicated. Recommendations received by the 20th of each month are distributed the first week of the following month.

Furthermore, I understand that the Jewish Community Foundation’s Board of Directors has full discretion as to the recipients of distributions from this Donor Advised Fund.
